How we help.
Your hospice team is made up of local professionals devoted to making this season as peaceful as possible for your family.
Your dedicated nurse and aide work closely with our medical director to create and carry out a custom care plan that often includes medication and therapies to manage pain. Your social worker helps you navigate non-medical needs, including living wills, financial considerations, community resources, and planning final arrangements. And finally, our chaplain can help you navigate pertinent questions, find peace in the process, and provide ongoing support for your family.
